Rental Form:

If this is your initial rental experience with us, there are some requirements that must be met. Please ensure you provide the following:

• A completed credit application.
• Evidence of insurance, with Columbus Equipment named as Loss Payee and Additional Insured regarding the rented or leased equipment. You must have a minimum of $1,000,000 in General Liability coverage, as well as sufficient coverage for the specific equipment being rented.
• A Certificate of Insurance, with Columbus Equipment listed as the certificate holder.
